Different conact form

I would like a contact form that would look something like that:

(Ignore the background)
And when administrator would receive a PM, it would be that way:

Email: [EMAIL WRITTEN IN EMAIL FIELD]
User name: [NAME WRITTEN IN USERNAME FIELD]
Link and comments: [TEXT WRITTEN IN TEXT AREA]

or at least saying also email and username (and of course, message - text area).

At the contact form,when he sends you the pm,you can see his mail.The skin title & comments fields are exactly the same as the Name fields.Would you like me to code it for you?
